Request for Invitation

Please use the form below to request an official letter of invitation for the upcoming Congress. An invitation letter is not required for attendance but will be needed if you are applying for a visa. Letters are emailed only to those individuals who will be attending the meeting. If you require a hard copy via express mail, there will be a $12.00 (USD) charge. Please contact Lindsay McCall at + (573) 882-9554 to arrange payment to have a copy of your letter of invitation mailed to you.

International Attendees
As of October 26, 2005, all countries must issue passports with digital photos or the required chip (e-passport) containing biometric information about the individual. However, anyone issued a machine-readable passport prior to October 26, 2005, will be “grandfathered in” and allowed to visit the United States until the passport expires.
